Swim Registration Page
Dear Parents,

We have created this page to help us with registering all swimmers.  You will need to complete a form for each swimmer.  By pre-registering you will allow us to be more efficient and have a better idea of the number of swimmers we have.  You do not have to do the online payment to register.  You may bring a check with your forms to the pool during one of the registration times.

You will need to print out the following forms, complete them and bring them to the registration.

WE MUST HAVE ALL FORMS PRIOR TO YOUR CHILD ENTERING THE WATER.  IF THE REGISTRATION FORMS ARE NOT COMPLETE, YOUR CHILD WILL NOT BE ALLOWED TO SWIM.

1) Mast Contract

2) Medical Authorization Form

3) USA Swimming Seasonal Registration Form

4) MAST Financial Form

5) Your receipt showing your initial $250 deposit that will be emailed to you once you complete the pre-registration information -- If you elect to pay online.
